Follow the checklist in the Credentials Review Requirements shown below.

Keys for the credentials review:

- Have your Association Registrar run a copy of the Credentials Verification Sheet form right after January 1st. The database should be up to date with the basic information required on that sheet.

- If the sheet does not have the latest C.E.P. information for a coach, does not show that a Coaching Module was completed or that SafeSport was completed, bring a hard copy as proof of compliance. You can get a copy of these items on the www.usahockey.com web site if needed. If they have not completed these items by December 31st, they can no longer coach the team for the balance of the season. It is the coach's responsibility to make sure that they are in compliance with these USA Hockey coaching requirements.

- If the Background Screening box is not checked on the Credentials Verification Sheet, bring proof of compliance (the receipt that you have submitted for the background check; not the "green" response). This can be obtained from the www.maha.org web site.

- In advance of your credentials review, go through all of your game sheets to make sure that: a) you have all of them & b) score sheets clearly show that players or coach(s) have sat any game misconducts.

Credential Check In will be held on Sunday, January 8th at SCS Civic Arena in the upstairs area above Gardens Rink.

Each division is asked to come at their designated times:

Squirt 8am - 9am
Pee Wee 9am - 10am
Bantams 10am - 11am
Midgets 11am - 12pm
HS/JV/Carryover 12pm - 1pm

Please do your best to have all the necessary information for check in ready to go and arrive during your division timeframe.

Please ensure you have:
Game Log & Scoresheets for EVERY game
Credentials Sheet (filled out)
Credentials Verification Sheet (filled out)
Official USA Hockey Team Roster
Consent to Treat forms for all players and coaches (IF NATIONAL BOUND ONLY)
